Providence Health & Services Wound and Ostomy Coordinator (RN) in San Pedro, California

Description:

Providence is calling a Wound and Ostomy Coordinator (RN) (Full time/ Day shift ) to rovidence Little Company Of Mary San Pedro Hospital in San Pedro, CA.

We are seeking a Wound and Ostomy Coordinator (RN) who will serve as a clinical expert, consultant and facilitator for Providence Little Company of Mary Service Area in the assessment, planning, implementation and evaluation of patient care practices, policies, and educational activities related to selected disorders of the gastrointestinal, genitourinary, and integumentary systems, such as complex draining wounds, ostomies, fistulae, dermal ulcers, tubes and incontinence. Responds to patients and families with knowledge of age specific needs within Division of Nursing and support the Mission Statement and Core Values of Little Company of Mary.

In this position you will:

  • Education & Staff Development, Clinical Expert:

  • Collaborate with physicians, nursing directors, managers and staff including Home Health to assess staff and patient and family learning needs

  • Plan and implement educational activities to meet identified learning needs of patient /families using principles of adult education

  • Plan and implement educational activities to enhance clinical development and proficiency of clinical staff

  • Facilitate inservices related to new equipment, products, practice/process changes, and patient safety/quality improvement

  • Develop and utilize evaluative processes to determine effectiveness and satisfaction related to patient/family and clinical staff education educational activities

  • Actively participate on the Providence Health and Services Southern California (PHS CA) Regional Skin Wound Ostomy Excellence Council

  • Evaluate national guidelines, best practices, and research findings for application to clinical policies and education activities related to the care of:

  • Patients with colon disorders resulting in the formation of an ostomy

  • Patients with conditions requiring wound management and containment

  • Patients having impaired/altered skin integrity

  • Collaborate with PHS CA Region colleagues in the development of clinical policies, guidelines, and order sets

  • Provide consultation and direct assistance to nursing staff for issues related to patients with altered skin integrity, secondary pressure necrosis, drainage or incontinence

  • Make recommendations regarding the appropriate site for stoma placement with consideration given to patient’s anatomical markings, physical capabilities and lifestyle

  • Provide patient education content directly or via other clinical staff to patients including those undergoing ostomy and other surgeries and for post discharge needs

  • Participate in standing committees, task forces, meetings and activities to assist patient /family and staff development resulting in improved patient care outcomes

  • Participate in Interdisciplinary Rounds

  • Make visits to PLMMC Home Health patients on an ad hoc basis after consulting with Home Care nursing staff and leadership

  • Leadership/Supervision:

  • Serve as clinical expert for PLCMMCs and role model for nursing staff

  • Oversee the clinical practice of wound, ostomy, continence nursing intern

  • Collaborate with other skin care team members to ensure standardized clinical practices

  • Act as a change agent and demonstrates risk taking skills

  • Participate in the support of nursing orientation

  • Actively support the development of other members of the healthcare team

  • Documentation/Information Management:

  • Participate in the organization’s commitment to maintaining the confidentiality of patient information through maintenance of related policies and procedures

  • Utilize clinical information systems to support data and monitor outcomes

  • Maintain and submit required and statistical reports to appropriate clinical leaders

  • Adhere to the documentation system within Education and Professional Development Department of continuing education activities according to BRN and department guidelines

  • Demonstrate working knowledge of Hospital Information Systems

  • Development:

  • Maintain certification in area of expertise

  • Promote professional development via education activities that enhance practice and support performance improvement programs

  • Mentor and coach wound, ostomy, continence nursing intern

  • Is involved in professional organizations and relationships with WOC nurses in order to benchmark best practice

  • Promote continuing professional growth for self and others as an adjunct to excellence in practice

  • Performance Improvement/Quality and Safety:

  • Actively participate in performance improvement activities and work to correct deficiencies identified

  • Actively support the facility with regards to continuous compliance with all regulatory and performance improvement standards including, but not limited to TJC, Department of Health Services, CMS Core Measures, IHI initiatives, etc.

  • Maintain current knowledge regarding TJC standards for this position

  • Commit to and actively participates in ongoing compliance

  • Have knowledge of Tracer methodology

  • Provide the best care and service to every person, every time (PHS Quality Vision)

  • Provide care that is safe, timely, effective, efficient, equitable and patient/family centered (PHS Quality definition)

  • Use PHS performance improvement process for all performance improvement activities

  • Support, monitor, and trend key indicators related to wound, ostomy, and continence

  • Consistently seek methods and strategies to improve educational activities related to wound, ostomy, and continence

  • Budget Responsibilities:

  • Responsible for managing department resources prudently

Qualifications:

Required qualifications for this position include:

  • Bachelor's Degree within 1 year of hire

  • Current California Registered Nurse License

  • 1 year acute hospital experience in clinical specialty area

  • Experience in nurse-peer teaching and ability to communicate with all levels of staff

Preferred qualifications for this position include:

  • Master's Degree

  • Graduate preparation in adult education

  • Board certification as WOC Nurse by Wound, Ostomy, Continence Nursing Certification Board (CWOCN)
