Providence Health & Services RN MDS Coordinator - Sub Acute Care Center in San Pedro, California
Providence is calling a Full Time, Day Shift Sub Acute Registered Nurse Minimum Data Set Coordinator to Providence Little Company of Mary Sub Acute Care Center in San Pedro, CA.
The Sub Acute Care Center is a 125-bed licensed facility accredited by the Department of Health Services (DHS). It is one of the largest facilities in California providing care to ventilator and tracheostomy patients. Whenever possible, patients are gently weaned from ventilators.

In this position you will:
Be responsible for conducting and coordinating the MDS Assessment with the appropriate participation of health professionals.
Be knowledgeable of care required: Young Adult to Geriatric.
Required qualifications for this position include:
Associate degree in Nursing
Current CA State Registered Nurse ( RN ) license
Current BLS certification for Healthcare Providers from the American Heart Association
Minimum 2-years RN experience in long term care facility and knowledge of Title XXII regulations and OBRA standards.
Preferred qualifications for this position include:
Bachelors degree in Nursing ( BSN )
Recent (at least 1 year experience), hospital direct patient care or work experience in a Long Term Care, Sub Acute and/or TCU setting
