Providence Health & Services LVN Case Manager - Sub Acute Care Center in San Pedro, California
Providence is calling a LVN Case Manager ( Full Time / Day Shift ) to Providence Little Company of Mary Medical Center in San Pedro, CA.
In this position you will:
Work under the direction of an RN following assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client’s care.
Monitor and transition care under direction from the RN.
“Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality cost effective outcomes.” (CMSA, 2009)
LVNs work under the direction of an RN (B&P Code, Section 2859). Prior to making patient care assignments to LVNs or CNAs, the RN must conduct a comprehensive patient assessment [Title 22, Section 70215(a)].
The LVN is not an independent practitioner (B&P Code, Section 2859).
The LVN does not initiate, evaluate or change the patient's treatment/nursing care plan. The LVN participates in planning, executes interventions in accordance with the treatment/nursing care plan, and contributes to evaluation of individualized interventions related to the treatment/nursing care plan (Title 16, Section 2518.5).
Required qualifications for this position include:
California State Licensed Vocational Nurse License ( LVN )
Two to three year’s acute hospital experience, including one year of supervisory experience.
Basic Life Support ( BLS ) certification within 90 days of hire
Strong knowledge base or experience in specialty area assigned.
Basic computer skills, Word, Excel, Access and PowerPoint.
Knowledge regarding State andFederal regulations, HMO, PPO and Utilization processes
Knowledge of Discharge planning and Case Management
Knowledge of principles and practices of Utilization and Quality Management
Knowledge of CPT, IDC-9 Coding
Excellent communication skills, both oral and written
Excellent customer service skills
Strong problem-solving skills and flexibility
Ability to plan, organize, prioritize, evaluate and document
Excellent presentation skills
Preferred qualifications for this position include:
