Providence Health & Services Community Health Worker Academy Intern in San Pedro, California
Providence St. Joseph Health is calling Temporary Community Health Worker II (On Call, Variables) to our location in San Pedro, CA.
We are seeking a Community Health Worker Academy Intern to be responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW-II will provide job duties within at least one of the following tracks:
In this position you will have the following responsibilities:
The Community Health Worker (CHW) Academy Intern is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.
The CHW Academy Intern provides primary job duties within at least one of the following tracks:
a) Enrollment: CHW Academy Interns in this track will provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provide one-on-one health insurance and/or CalFresh enrollment application assistance.
b) Navigation/Linkage to Resources: CHW Academy Interns in this track will provide individualized assessments in clinical settings such as emergency departments, primary care clinics, or in a community-based health center setting, then referring them to the appropriate programs and resources, and providing follow-up calls to make sure the needs are met. This includes placing follow up calls to patients tested positive for COVID-19 to ensure they contact their primary care provider for further recommendations concerning treatment and level of care.
Required qualification for this position include:
H.S. Diploma or GED
An interest in the health care field and becoming a Community Health Worker Or equivalent educ/experience
1 year Experience working with the public or with community groups in performing duties such as interviewing clients or patients concerning health or social service matters, answering questions, and giving information about health, mental health, and/or social services to clients or patients. Experience working in a health care setting such as a clinic, health center, hospital, etc.
•CA driver’s license
Preferred qualifications for this postiion include:
- Bi-lingual English /Spanish, highly preferred.
The Community Health Worker Academy is an important opportunity to develop state-of-the-art training for CHWs and to build a regional approach to expansion with this workforce that serves a variety of needs of under-resourced communities
The health workers will be integrated into the teams in partnering clinics and will help patients navigate resources such as CalFresh, Covered CA and Medi-Cal. One goal is to improve access to preventive and medical care services rather than the use of costly hospital emergency care in nonemergency situations.
