We are seeking a Community Health Supervisor - RN who will be responsible for the leadership and day-to-day management of assigned Community Health programs that provide clinical servies. Responsiblities include supervising and leading caregivers to ensure that patient/client services are provided in a safe and timely manner; ensuring compliance with internal and external policies, procedures and standards; conducting system analysis to identify and implement improvements. He/sh also develops and maintains intentional relationships with community partners and facilities partnerships with Providence staff, school districts, churches, clinics, local government and businesses, community groups, funding agencies and community members.

In this position you will:

  • Responsible for effective and efficient operations of clinical services

  • Supervise daily functions of the programs, monitors productivity of caregivers to ensure proper supervision and equitable distribution of caregivers workload.

  • Conduct regular caregivers meetings to facilitate communication.

  • Works to improve the patient/client experience and resolve concerns related to quality and expectations of care/interactions.

  • Perform nursing duties when required.

  • Review monthly statistical reports and utilizes data to improve operations. Implements actions plans to achieve department, grant and/or regulatory goals

  • Leadership/Supervision

  • Supervise Community Health staff and volunteers, including setting priorities, hiring, scheduling, training, evaluation, discipline, termination, etc.

  • Monitor and provide timely evaluations of employees’ job performance and competencies in accordance with job description and employee’s goals and objectives, including observation of employee in their work environment. Identifies ongoing training needs of caregivers and develops plans for getting these needs addressed.

  • Ensure that caregiver’s Kronos is accurate and complete.

  • Participate and provides feedback performance evaluations for caregivers in a timely manner.

  • In conjunction with Human Resources and manager, counsels caregivers on performance related issues.

  • Safety, Quality Assurance and Improvement

  • Ensure adherence to established department policies, procedures and protocols. Includes compliance with hospital, department and regulatory standards including, but not limited to, safety, infection prevention, quality management, environmental, confidentiality, payroll/time keeping practices and patient care.

  • Responsible for the maintenance of all policy and procedure / safety logs/ regulatory logs.

  • Develop and monitors quality improvement action plans.

  • Ensure that caregivers perform in a safe manner and that a safe environment is maintained for caregivers, patients/clients.

  • Obtain necessary knowledge and skill needed to maintain competence, certifications and stay abreast of current nursing standards

  • Advise department leadership on clinical policies, procedures, and standards to be considered in developing new programs or services

  • Stewardship/Fiscal Management:

  • Provide input to department leadership for preparation of program’s budget to meet the needs of clinical operations

  • Proactively responds to fiscal changes by adjusting operations to ensure productivity and expense standards are met.

  • Demonstrates effective communication and decision-making skills:

  • Promote positive and effective working relationships within the work environment.

  • Create and maintains positive and productive relationship with external partners, including but not limited to, school districts, grantors, county agencies, sub-contractors, and Community Based Organizations.

Qualifications:

Required qualifications for this position includes:

  • Bachelor's Degree in Nursing

  • CA RN

  • BLS

  • 1 year experience in clinical care

  • Strong written and verbal communication and interpersonal skills

  • Management and leadership skills, especially ability to organize and conduct meetings.

  • Ability to develop and maintain effective teams.

  • Problem solving, decision making, and implementation abilities.

  • Ability to successfully interact with community agencies, schools and grantors

Preferred qualifications for this position include:

  • Master's Degree in Nursing, Public Health, Health Service Administration, Community Health, or related field

  • 1 year in Experience in a supervisory role.

  • Bilingual English/Spanish
